Description
The National University Health System (NUH) seeks a dedicated Case Management Officer (WEHS) to join the Integrated Program for Management of Depression in Adolescents (IPMDA). This contract role is pivotal in delivering holistic, integrated care to adolescents experiencing depression, ensuring seamless coordination across various healthcare services.
About the Role
You will play a central role in the IPMDA team, contributing directly to the program's mission of providing comprehensive care for adolescents with depression. Your work will involve assessing individual needs, developing personalized care plans, and facilitating access to appropriate interventions and support services within the NUH framework.
Key Responsibilities
- Conduct thorough assessments of adolescents presenting with depression to determine their specific needs and risks.
- Develop, implement, and monitor individualized care plans in collaboration with adolescents, families, and the multidisciplinary IPMDA team.
- Coordinate and facilitate access to relevant services, including psychological therapies, medical consultations, educational support, and community resources.
- Maintain accurate and confidential records of assessments, care plans, and service coordination activities.
- Act as a liaison between adolescents, families, and various healthcare providers to ensure continuity of care.
- Contribute to the ongoing evaluation and improvement of the IPMDA program through feedback and data collection.
Requirements
- Minimum Bachelor's degree in a relevant field such as Psychology, Social Work, Nursing, or a related healthcare discipline.
- Proven experience (minimum 1 year) in case management, care coordination, or a similar support role within a healthcare or community mental health setting.
- Strong interpersonal skills, empathy, and the ability to build rapport with adolescents, families, and diverse professionals.
- Excellent communication and active listening skills, both verbal and written.
- Ability to work effectively within a multidisciplinary team and collaborate with various stakeholders.
- Understanding of adolescent development and mental health issues, particularly depression.
- Proficiency in relevant software and documentation systems.
